fa توسعه یک تکنیک ترکیبی یکپارچه نوین برای تجزیه‌وتحلیل حوادث صنعتی مبتنی بر انحرافات سیستماتیک و علل ریشه‌ای Development of a Novel Integrated Hybrid Technique for Industrial Accident Analysis Based on Systematic Deviations and Root Causes ایمنی Safety پژوهشي Research زمینه و هدف: حوادث شغلی در صنایع پیچیده چالش‌های عمده‌ای ایجاد می‌کنند و تکنیک‌های سنتی تحلیل محدودیت‌هایی مانند تمرکز ناکافی بر انحرافات سیستماتیک، عوامل انسانی-سازمانی و وابستگی‌ها دارند. این مطالعه تکنیک نوین Industrial Root-cause Analysis & Judgment (IRAJ) را بر پایه تحلیل تغییرات (Change-Based Analysis) و چارچوب 6M (انسان، ماشین، روش، مواد، مدیریت، محیط) توسعه داد تا دقت، جامعیت و کاربرد تحلیل حوادث را بهبود بخشد. روش بررسی: تکنیک IRAJ با ترکیب تحلیل تغییرات و 6M در 6 گام اصلیتوسعه یافت. معیارهای ارزیابی (سیستمی بودن، عمق تحلیل، سرعت، بی‌طرفی، پشتیبانی از اقدامات اصلاحی، یادگیری سازمانی، کاربرد صنعتی) از مرور ادبیات استخراج شد. پنل خبرگان مطالعه شامل 10 متخصص HSE با حداقل 10 سال تجربه بود. معیارها با ANP (با در نظر گرفتن وابستگی‌ها) تعیین و رتبه‌بندی تکنیک‌ها با VIKOR انجام گردید. پنج حادثه نمونه از صنایع متنوع تحلیل شد و امتیازات بر اساس نظر خبرگان محاسبه گردید. یافته‌ها: تکنیک IRAJ با امتیاز 0.34 در رتبه اول قرار گرفت (FAM: 0.26، Tripod-β: 0.22، SCAT: 0.18، CBA: 0.16، ECFC: 0.14، RCAT: 0.10). نسبت به روش‌های مرجع، عمق تحلیل 28%، پوشش عوامل انسانی-سازمانی 31%، پشتیبانی از اقدامات اصلاحی 35% و کاهش وابستگی به قضاوت تحلیل‌گر 29% افزایش یافت. همچنین بیشترین علل ریشه‌ای در مدیریت (28%) و روش (24%) شناسایی شد. نتیجه‌گیری: تکنیک IRAJ رویکردی ساختاریافته و تغییرمحور ارائه می‌دهد که تحلیل حوادث را جامع‌تر و دقیق‌تر می‌کند. علاوه بر این، یافته‌ها نشان می‌دهد که به کارگیری روش ANP–VIKOR می‌تواند یک چارچوب علمی و استاندارد برای ارزیابی و رتبه‌بندی تکنیک‌های تحلیل حوادث فراهم کند، به طوری که معیارهای مهم همزمان مورد سنجش قرار گیرند و تصمیم‌گیری به صورت کمی و شفاف انجام شود.   <div style="text-align: justify;">Background and Aims: Occupational accidents in complex industries present significant challenges, and traditional analysis techniques have limitations, including insufficient emphasis on systematic deviations, human-organizational factors, and interdependencies. The primary objective was to evaluate novel Industrial Root-cause Analysis & Judgment (IRAJ) technique using the ANP–VIKOR multi-criteria decision-making method and to compare it with conventional techniques. Method: The IRAJ technique was developed by integrating Change-Based Analysis with the 6M framework through six main steps. Evaluation criteria—including systematic nature, analysis depth, speed, neutrality, support for corrective actions, organizational learning, and industrial applicability—were derived from a comprehensive literature review. The expert panel comprised 10 HSE specialists. Criteria weights were determined using ANP (considering interdependencies), and the techniques were ranked using VIKOR. Results: The IRAJ technique ranked first with a score of 0.34 (FAM: 0.26, Tripod-β: 0.22, SCAT: 0.18, CBA: 0.16, ECFC: 0.14, RCAT: 0.10). Compared to the reference methods, the depth of analysis increased by 28%, coverage of human-organizational factors improved by 31%, support for corrective actions rose by 35%, and reliance on analyst judgment decreased by 29%. Furthermore, the most frequent root causes were identified in management (28%) and method (24%). Conclusion: The IRAJ technique provides a structured, change-oriented approach that enhances the comprehensiveness and accuracy of accident analysis. Furthermore, the findings indicate that applying the ANP–VIKOR method offers a scientific and standardized framework for evaluating and ranking accident analysis techniques, ensuring that critical criteria are assessed simultaneously and that decision-making is conducted quantitatively and transparently.
