fa وضعیت اجتماعی- اقتصادی و شدت آسیب تصادفات رانندگی: یک مطالعه مقطعی Socioeconomic status and severity of traffic accident injuries: a cross-sectional study اقتصاد سلامت Health Economics پژوهشي Research زمینه و هدف:  حوادث ترافیکی جاده¬ای یکی از مشکلات جدی بهداشت عمومی در دنیا می¬باشد که پیشگیری از آن ضروری است. وضعیت اجتمـاعی-اقتصادی از فاکتورهای مهم مرتبط با حوادث ترافیکی است. میزان بروز و مرگ و میر ناشی از حوادث ترافیکی نیز بین کشورهای مختلف و حتی در داخل یک کشور متفاوت است. بنابراین هدف مطالعه حاضر بررسی ارتباط شدت آسیب تصادفات رانندگی با عوامل اجتمـاعی-اقتصادی بود.. روش بررسی: این مطالعه یک مطالعه مقطعی است که بر روی 310 نفر از رانندگان بستری ‌شده به دنبال تصادف در بیمارستان امام خمینی ارومیه در سال 1399 انجام شد. مقیاس شدت آسیب ISS (Injury Severity Score) و مدل آماری رگرسیون لجستیک رتبه‌ای برای این مطالعه استفاده شد. یافتهها: دادهها نشان داد که بین شدت آسیب و عوامل اجتماعی-اقتصادی از جمله وضعیت تأهل (306/2=OR، 024/0=P)، تحصیلات زیردیپلم (066/19=OR، 013/0=P) و دیپلم (549/5=OR، 011/0=P)،  درآمد ماهانه خانوار برای 5/12 تا 25 میلیون ریال (87/4=OR، 068/0=P) و برای 25 تا 5/37 میلیون ریال (306/2 =OR، 024/0 =P)، شغل کارگری (491/2 =OR، 088/0=P)، ارزش پولی خودرو برای صفر تا 550 میلیون ریال (986/13=OR، 001/0) و برای 550 تا1100 میلیون ریال (284/3=OR، 016/0=P) و همچنین سهم هزینه های بهداشت و درمان برای صفر تا20٪ (389/22=OR، 001/0=P) و برای 20٪ تا40٪ (664/3=OR، 001/0=P) رابطه مثبت و معناداری وجود داشت، ولی بین شدت آسیب و سن (889/0=OR، 367/0=P)، جنسیت (391/1=OR، 598/0=P) و ارزش پولی مسکن (همه دستهها) ارتباط معنیداری مشاهده نشد. نتیجهگیری:  داده¬ها حاکی از آن است که بین اکثر عوامل اجتماعی-اقتصادی مورد پژوهش و شدت آسیب رابطه وجود دارد. بر اساس نتایج بدست آمده، برنامه¬های مداخله¬ای درمورد وضعیت تأهل، ارتقاء سطح تحصیلات، افزایش درآمد ماهانه خانوار، بهبود وضعیت کاری و شغلی، ارتقاء ارزش پولی و ایمنی خودرو و افزایش سهم هزینه¬های بهداشتی و مراقبتی افراد از کل مخارج زندگی، توصیه می-شود.   Background and aims: Road traffic accidents are one of the serious public health problems in the world that need to be prevented. Socio-economic status is one of the important factors related to traffic accidents. The incidence and mortality rate of traffic accidents also vary between different countries and even within a country. Therefore, the aim of this study was to investigate the relationship between the severity of traffic accident injuries and socio-economic factors. Methods: This study was a cross-sectional study involving 310 drivers caring following an accident in Imam Khomeini Hospital in Urmia, Iran in 2020. The trauma severity scale of ISS ((Injury Severity Score) and the statistical model of rank logistic regression were used for this study. Results: Data showed a positive and significant relationship between the severity of injury and socioeconomic factors including marital status (OR = 2.306, P = 0.024), education for high school (OR = 19.066, P = 0.013), and diploma (OR = 5.549, P = 0.011), household monthly income for 12.5 to 25 million Rials (OR = 4.87, P = 0.068), and for 25 to 37.5 million Rials (OR = 4.46, P = 0.079), occupation (OR = 2.491, P = 0.088), car monetary value for 0-550 million Rials (OR = 13.986, P = 0.001), and for 550-1100 million Rials (OR = 3.284, P = 0.016),  as well  the share of health care costs for 0-20% (OR = 22.389, P = 0.001) and  for 20%-40% (OR = 3.664 P = 0.001), while was not a significant association between the severity of injury and age (OR =0.889, P = 0.367), gender (OR = 1.391, P = 0.598), and monetary value of housing. Conclusions: The data indicate that there was a relationship between most of the socio-economic factors and the severity of injury.
