fa ارائه روشی جدید جهت تجزیه و تحلیل علل ریشه‌ای حوادث ایمنی Safety پژوهشي Research <span style="font-size:11pt"><span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><span style="font-family:Calibri,sans-serif"><b><span lang="FA" style="font-size:12.0pt"><span b="" lotus="" style="font-family:">مقدمه</span></span></b><b><span b="" lang="FA" lotus="" style="font-family:">:</span></b><span b="" lang="FA" lotus="" style="font-family:"> امروزه بسیاری از سازمان‌ها و شرکت&shy;های ایرانی روش&shy;ها و دستورالعمل&shy;های مختلفی را جهت تجزیه و تحلیل حادثه اتخاذ نموده&shy;اند، اما هنگامی که یک حادثه به وقوع می&shy;پیوندد تنها با یک یا چند خط و به شکل ساده و توصیفی تجزیه و تحلیل حادثه را انجام می‌دهند. هدف از انجام این تحقیق، بررسی و شناسایی فاکتورهای ریشه&shy;ای علت حادثه و نیز ارائه تکنیکی جهت تجزیه و تحلیل دقیق و جامع&shy;تر حوادث با شدت و فرکانس بالا است.&nbsp; </span></span></span></span></span></span><br> <span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><b><span lang="FA" style="font-size:12.0pt"><span b="" lotus="" style="font-family:">مواد و روش&shy;ها</span></span></b><b><span b="" lang="FA" lotus="" style="font-family:">:</span></b><span b="" lang="AR-SA" lotus="" style="font-family:"> پژوهش حاضر از نوع کاربردی است. در گام اول گزارشات درس آموخته&shy;های حوادث در صنعت حفاری از طریق روش بررسی اسناد مورد بررسی قرار گرفت. فاکتورهای شناسایی شده در گام اول با روش </span><span dir="LTR" new="" roman="" style="font-family:" times="">AHP-Fuzzy</span><span b="" lang="FA" lotus="" style="font-family:"> وزن&shy;دهی و میزان تأثیر آن&shy;ها در وقوع حادثه طبق نظر خبرگان تعیین گردید. در ادامه با طراحی جداول راهنما برای هر فاکتور، </span><span b="" lang="AR-SA" lotus="" style="font-family:">یکی از حوادث منجر به فوت در یک شرکت حفاری نفت انتخاب شد. سپس طبق مدل تعریف شده مورد تجزیه و تحلیل و ریشه&shy;یابی قرار گرفت.</span><span lang="AR-SA" style="font-family:"B Lotus""></span></span></span></span><br> <span style="font-size:11pt"><span style="line-height:normal"><span style="page-break-after:avoid"><span style="tab-stops:center 225.65pt left 376.8pt"><span style="direction:rtl"><span style="unicode-bidi:embed"><span style="font-family:Calibri,sans-serif"><b><span lang="FA" style="font-size:12.0pt"><span b="" lotus="" style="font-family:">نتایج</span></span></b><b><span b="" lang="FA" lotus="" style="font-family:">:</span></b><span b="" lang="AR-SA" lotus="" style="font-family:"> براساس نتایج بدست آمده از بررسی اسناد، 29 فاکتور شامل فاکتورهای سازمانی، شغلی و محیطی در ارتباط با علل ریشه‌ای حوادث شناسایی شدند. در بین معیارهای سطح دوم یعنی عوامل سازمانی، شغلی و محیطی عامل سازمانی با کسب بار وزنی 533/0 بیشترین و معیارهای محیطی با بار وزنی 0198/0 کمترین تأثیر را بر وقوع حوادث </span><span b="" lang="FA" lotus="" style="font-family:">داشتند</span><span b="" lang="AR-SA" lotus="" style="font-family:">. براساس نتایج به دست آمده مشخص گردید که عدم مهارت شغلی از معیارهای شغلی علت اصلی وقوع حادثه و عدم تناسب شغلی و آموزش ایمنی از جمله زیرمعیارهای رده&shy;های دوم و سوم جزء علل ریشه‌ای رخداد مورد مطالعه به شمار می&shy;آیند.</span></span></span></span></span></span></span></span><br> <span style="font-size:11pt"><span style="line-height:normal"><span style="direction:rtl"><span style="unicode-bidi:embed"><span style="font-family:Calibri,sans-serif"><b><span lang="FA" style="font-size:12.0pt"><span b="" lotus="" style="font-family:">نتیجه گیری</span></span></b><b><span b="" lang="FA" lotus="" style="font-family:">:</span></b><span b="" lang="FA" lotus="" style="font-family:"> در مطالعه حاضر سه فاکتور سازمان، محیط و فرد براساس تئوری عوامل انسانی در ریشه&shy;یابی حوادث صنعتی در صنعت حفاری مورد بررسی قرار گرفتند. تحلیل</span> <span b="" lang="FA" lotus="" style="font-family:">داده&shy;های</span> <span b="" lang="FA" lotus="" style="font-family:">مطالعه</span> <span b="" lang="FA" lotus="" style="font-family:">نشان</span> <span b="" lang="FA" lotus="" style="font-family:">داد</span> <span b="" lang="FA" lotus="" style="font-family:">که</span> <span b="" lang="FA" lotus="" style="font-family:">ابزار</span> <span b="" lang="FA" lotus="" style="font-family:">طراحی‌شده،</span> <span b="" lang="FA" lotus="" style="font-family:">روشی</span> <span b="" lang="FA" lotus="" style="font-family:">عینی</span> <span b="" lang="FA" lotus="" style="font-family:">و</span> <span b="" lang="FA" lotus="" style="font-family:">ساده</span> <span b="" lang="FA" lotus="" style="font-family:">جهت</span> <span b="" lang="FA" lotus="" style="font-family:">ریشه&shy;یابی</span> <span b="" lang="FA" lotus="" style="font-family:">علل حوادث است و می&shy;تواند به‌عنوان یک روش کاربردی و جامع جهت تجزیه و تحلیل</span> <span b="" lang="FA" lotus="" style="font-family:">حوادث سازمانی مورد استفاده قرار گیرد. </span></span></span></span></span></span><br> <br> &nbsp; <span style="font-size:11pt"><span style="line-height:normal"><span style="font-family:Calibri,sans-serif"><strong>Introduction</strong><span new="" roman="" style="font-family:" times="">: Today, many organizations and companies have some methods and instructions to analyze the causes of accidents, but when an accident occurs, only a simple approach is implemented to analyze the causes of accidents.&nbsp;This study aims to investigate and identify the root causes of accidents in the country&#39;s drilling industry and provide a technique for an accurate and more comprehensive analysis of accidents with high intensity and frequency in this industry.</span></span></span></span><br> <span style="font-size:11pt"><span style="line-height:normal"><span style="font-family:Calibri,sans-serif"><strong>Materials and Methods</strong><span new="" roman="" style="font-family:" times="">: The present study was conducted in 4 main stages. In the first step, the lessons learned from accidents in the drilling industry were studied through the method of document review. The factors identified in the first step were weighted by the AHP-Fuzzy method and their impact on the occurrence of the accident was determined according to the expert&rsquo;s opinion. One of the fatal accidents in an oil drilling company was selected and then the root causes of the accident were analyzed according to the developed model.</span></span></span></span><br> <span style="font-size:11pt"><span style="line-height:normal"><span style="font-family:Calibri,sans-serif"><strong>Results</strong><span new="" roman="" style="font-family:" times="">: Based on the results of the review of documents, 29 factors including organizational, occupational, and environmental factors were identified as the root causes of accidents. Among the second level criteria, the organizational factor with the weighting load of 0.533 had the highest and the environmental criteria with the weighting load of 0.0198 gained the lowest weighting load. Based on the results, it was found that job incompetence is one of the main causes of accidents, and job incompatibility and safety training are among the second and third sub-criteria and are among the main causes of the accident.<br> <strong>Conclusion</strong>: In the present study, three factors of organization, environment, and individual based on the theory of human factors as the root causes of industrial accidents in the drilling industry were investigated.
